Articles - Articles – Effect of the previous use of desensitizer in the result of whitening care with Total Blanc H35

    9.12.11 - Nova DFL

    SUMMARY
Dental whitening is based upon the penetration of oxygen molecules in the dental structure coming from the carbamide or hydrogen peroxides, which dissolve the pigments by oxidation. The use of desensitizers that obliterate tubules forming a protection barrier is considered as a factor reducing the performance of the whitening agents.
The purpose of this article is reporting two clinical cases, with follow up of 9 months, where a desensitized based upon sodium fluorite 2% was previously applied to whitening with hydrogen peroxide 35% (Total Blanc, Nova DFL).
Female patients, 20 and 21 years of age, had naturally pigmented teeth in A3 color, having reported never having previously bleached teeth. Before cleaning, in both cases was applied a gel consisting of sodium fluorite 2%, during 15 minutes, followed by whitening with application of the whitening gel in a single session, during 45 minutes. In both cases the result after whitening achieved A1 color, maintained stable during the 9 months of follow up. After 9 months, the patients where again submitted to a new whitening session, visually noticing more whitening, even though the automatic color selection by the Easyshade (Vita) device continued recording A1 color.
We may conclude that the use of desensitizing gel based upon sodium fluorite did not affect the whitening result, reducing the chances of sensitivity during and after the clinical procedures.

    INTRODUCTION
Tooth color is predominantly represented by the dentine color and the changes may be differentiated by ethology, aspect, location and severity. In most cases, the color change occurs due to the presence of pigments diffused in the dental substrate, enamel and/or dentine. Less severe extrinsic spots may be removed by polishing the tooth structure. For those that are more difficult to eliminate due to higher grade of darkening and for intrinsic spots, there are various whitening techniques that may be used for vital and non-vital teeth. One of the inconveniences that may appear during and after tooth whitening is sensitivity, which happens maximum for every two to three patients and varies from patient to patient, classified from light to moderate, but in some cases significant discomfort may occur. In spite of the use of gel with higher concentration presenting quicker action the increase in the sensitivity level of the patient is remarkable and difficult to predict. Sensitivity occurs by the penetration of the whitening agent in the small dental channels in contact with the nerve ends of the teeth, causing an irritation process. This is usually reversible and solved on its own, after treatment and/or by using desensitizing agents. There are various desensitizing agents in the market with different action mechanisms. There is a large concern on the efficacy of the whitening process with previous use of these agents since many of the same act by forming a mineral layer on the tooth structure, which virtually would bar the passing of the whitening substance. However, studies attest that there is no interference in the efficacy and also attest the reduction of the sensitivity. Most of the existing papers indicate its use after the treatment, when there is sensitivity and during the treatment. In the application before the teeth whitening, Tay et AL demonstrated that the use of potassium nitrate 5% associated to sodium fluorite 2% did not affect the efficacy of the whitening with hydrogen peroxide 35% and reduced the sensitivity of the patients. Similarly, Zielbolz checked that the use of potassium fluorite 0,5% associated to methacrylate did not affect the whitening with hydrogen peroxide 6% applied twice a day (10minutes per application), during 7 days. The objective of this article is reporting two clinical cases, with follow up of 9 months, where a desensitizing based upon sodium fluorite 2% was applied previous to whitening with hydrogen peroxide 35% (Total Blanc, Nova DFL).

    Clinical case – Case 01
Patient E. S.S., female, 21 years of age, had naturally pigmented teeth in A3 color (Figure 1), having reported never having previous teeth whitening. The treatment was professional teeth whitening with hydrogen peroxide 35% (Total Blanc H35, Nova DFL, RJ), in a single session, after using desensitizing consisting of Sodium fluorite 2%.
